Terms and Conditions for Orhun Medical Gift Certificates

1. General Provisions

1.1. A Gift Certificate (hereinafter referred to as the “Certificate”) is a document granting its holder the right to receive a service* within the network of medical centers of LLP “Orhun Medical” (hereinafter referred to as the “Company”) in the amount equivalent to the nominal value of the Certificate as per the current price list at the time of redemption.

      • The Certificate includes an individual number, purchase date, validity period, and nominal value.
      • The Certificate Buyer refers to any individual or legal entity that has paid an amount equal to the nominal value of the Certificate for medical services. The
      • Certificate can be transferred to a third party for service redemption.
      • The Certificate Holder refers to the individual who possesses and presents the
      • Certificate for redemption.
      • Sample Certificates are approved in the Appendices to this Regulation.
      • Certificates cannot be exchanged for cash.

2. Rules for Acquiring Certificates

2.1. The Company may issue the following types of Certificates:

2.1.1. Standard Certificates (Paid Basis)

        • Standard Certificates can be purchased on a paid basis via cash and/or non-cash payments and redeemed for services provided by the Company’s medical centers at the time of presentation.
        • The Certificate Buyer must pay an amount equal to the nominal value in cash or transfer funds via bank payment to the Company’s account.
        • A fiscal receipt is issued to individuals upon purchase, while legal entities receive a transfer certificate and fiscal receipt for cash payments.

2.1.2. Marketing Certificates

        • Marketing Certificates may be issued to Company partners for strengthening cooperation or for promotional purposes to enhance brand awareness, under the terms outlined in a specific agreement with the Partner.
        • The design, nominal value, and usage terms of Marketing Certificates are defined in the Appendices to this Regulation.

2.1.3. Other Certificates

        • The Company may issue other types of Certificates as necessary, subject to approval by the General Director’s order.

*PET/CT services are provided in accordance with the legislation of the Republic of Kazakhstan.

3. Certificate Value

3.1. The nominal value of the Certificate is indicated on the Certificate itself. Certificates are available in the following denominations: 25,000 KZT, 50,000 KZT, 75,000 KZT, 100,000 KZT.

3.2. The nominal value grants the holder the right to obtain services equal to the Certificate’s value or partially pay for services exceeding the Certificate value.

3.3. Marketing Certificate values may be approved on a case-by-case basis.

4. Certificate Usage

4.1. The Certificate is activated on the day of sale/transfer and is valid for 12 months from the purchase/transfer date unless otherwise specified.

4.2. The validity period of the Certificate is not extendable.

4.3. Lost, stolen, or damaged Certificates cannot be restored, and funds are not refundable.

4.4. Unused Certificates past their validity period are non-refundable. Any remaining balance cannot be redeemed in cash.

4.5. The Company may refuse Certificate redemption if:

4.5.1. The holder does not present the Certificate or presents a falsified document.

4.5.2. The Certificate validity period has expired.

4.5.3. The Certificate is damaged beyond recognition.

4.5.4. The Certificate has already been used.

4.5.5. Services cannot be provided due to technical issues or other unforeseen circumstances (e.g., repairs, power outages). In such cases, the Company reserves the right to reschedule the service.

4.6. Named Certificates must be used by the named individual. Transfer to a third party requires prior notification to the call center, providing details of the Certificate and recipient.

4.7. Non-personalized Certificates can be freely transferred to third parties without identity verification.

4.8. Certificates are valid at all Company branches.

4.9. Certificates must be registered with the Company upon first use.

4.10. Certificates can be redeemed only within the city of initial registration

4.11. The Company is not responsible for how Certificates are transferred or used by buyers or third parties.

4.12. Remaining Certificate balances may be used for future visits within the validity period.

4.13. If the total cost of selected services exceeds the Certificate value, the holder must pay the difference in cash or via bank transfer.

4.14. Any balance remaining after a service purchase cannot be refunded but can be applied toward future services.

4.15. Multiple Certificates may be combined for a single transaction.

4.16. Damaged or suspicious Certificates will not be accepted.

5. Additional Provisions

5.1. The Certificate represents a public offer. These Regulations outline the essential terms of the public sales agreement using Certificates.

5.2. Buyers and recipients are made aware of these terms upon purchase/transfer. Purchasing/receiving a Certificate indicates agreement with these terms.

5.3. The Company records Certificates in its internal system prior to circulation and updates information as Certificates are redeemed
